We recommend always calling the member service number list on the front or back of your insurance card to verify that your particular plan allows you to choose one of our Nurse Practitioners as your Primary Care provider
One of our Primary Care Providers must be listed as your PCP the day of your visit
Our Billing Department will assist you with any necessary paperwork and other requirements relative to your insurance provider
