Our staff will be
pleased to assist you in any way possible and all efforts will
be made to accommodate your schedule.
Cancellations
If you are unable to keep an appointment,
we ask that you kindly provide us with at least 24 hours
notice. This courtesy, on your part, will make it possible to
give your appointment to another patient.
* During inclement weather, please call our office before
you come for you appointment.
Referrals
Referrals that are needed for outside specialists and other facilities, please
contact our office and ask for Extension 306. Please make sure to have the
specialist or facility name, phone number, and the reason for the visit.
Also, make sure to include your name, date of birth, and current insurance you
have.
Though we try and complete all referrals within 24-48. Please be advised
that out of network referrals, prior authorizations and other referrals may take
up to 1 week to process.
Prescription Refills
Patients currently established with one of Whole Life Health Care's Nurse
Practitioners and are in need of refilling their prescription, may call and ask
for extension 222. This is our designated Prescription Line. Please
make sure to leave your name, the spelling of you name, date of birth, which
prescription you would like filled, where you would like it filled, and the
location of the pharmacy as some towns have more than one.
Please be advised that prescriptions not needing a prior authorization will
be filled within 24 hours. Prescriptions needing a prior authorization
may take up to 2-3 business days. Messages left on the prescription line
after 4pm, may not be called in to the pharmacy until the next business day.
Billing / Payment
All billing inquiries are to be
directed to our office at
(603) 431-6677. Our staff is
prepared to answer any questions you may have regarding
payments.
Payment is due at the time of service
for all non-participating insurance companies. Co-payment is
also due at the time of service.
Patients are responsible for
notifying our office of any changes in their home billing
address/telephone numbers and with their primary/secondary
insurance coverage. Forms of payment accepted by the office
are cash, personal checks, money orders and the following
credit cards:
